  • Am I covered? To confirm that you are covered, login at www.nwadmin.com and select “Eligibility Status” near the top left side of the screen, or call the Administration Office using the phone number above.
  • Who in my family is covered? Enrollment is required for the employee/member and any eligible dependents before coverage can be used.If you are covered under Plan A, you have full family coverage, including dependent children up to 26 years of age.
    If you have Plan B, only the employee is covered.
    If you do not know which plan applies to you, contact the Administration Office at (844) 411-0786.

  • Am I eligible for benefits as a retiree? The Trust offers an Early Retiree Health Benefits Program designed to bridge the gap until you become eligible for Medicare.  Please see the Early Retiree Health Benefits section under Benefits Overview for details.The Trust also provides Medicare Eligibility Consulting to help you determine whether you are eligible for Medicare, and what your options are if you are not eligible.   • How do I get an ID card? Once you enroll, an ID card will automatically be mailed to the address you provided on the enrollment form. To request additional ID cards, contact the Administration Office online at www.nwadmin.com or using the phone number above.
  • How do I change my address? To change your address, submit an updated enrollment form to the Administration Office online at www.nwadmin.com or mail the completed form to the Administration Office:UNITE HERE NW Health Trust

Coverage

COBRA

  • What is COBRA? COBRA provides eligible employees and their covered dependents the option of continued health insurance coverage when an employee loses coverage due a reduction of work hours or job loss. COBRA coverage is provided on a self-pay basis.
  • How do I apply for COBRA? A COBRA notice and election form will automatically be sent to your address on file with the Trust if you experience a loss of coverage. Complete, sign and return the election form to set up your COBRA account. Once your election form is approved, a monthly billing for the cost of COBRA coverage will be sent to you automatically. Payment must be made and processed before COBRA coverage is provided.

Bills & Claims

  • How do I file a claim? If you use medical, dental, vision or pharmacy providers that are part of the Trust’s preferred networks, claims will be automatically filed on your behalf. The Trust will send you an Explanation of Benefits (EOB) detailing the payments made on your behalf and the provider will bill you directly for your share of the cost.If you use medical, dental, vision or pharmacy providers that are NOT part of the Trust’s preferred networks, you are responsible for filing claims for reimbursement using the applicable claim form (Aetna for medical claims) (EyeMed for vision) (Sav-Rx for medicines/prescriptions).
  • I don’t understand my bill, what should I do? Contact the provider to discuss the bill.
  • My claim was denied. What should I do? Review the reason/s listed on the Explanation of Benefits from the Trust. If you still have questions, contact the Administration Office to discuss.
  • I received a bill from my provider and insurance hasn’t paid yet. What should I do? Contact the provider to ensure they have submitted the claim to the Trust for processing. Have your ID card handy in case the provider needs information from the ID card.

How to Use Benefits

  • How do I find a doctor? To find a doctor, visit Aetna Directory of Healthcare Professionals, enter your zip code, and select “Aetna Choice POS II (open access) network” for the plan.
  • How do I find a dentist? Eligible participants enrolled in the Trust Traditional Dental Plan have two options: Cigna and Pacific Dental Alliance.To find a Pacific Dental Alliance dentist visit: www.pacificdentalalliance.comTo find a Cigna dentist: www.cignadentalsa.com and select “Cigna Dental PPO Shared Administration Plus” option.
  • How do I get my medicines and prescriptions? UNITE HERE Northwest Trust Fund works with Sav-Rx to provide cost effective prescription drug coverage. You have three options for filling your prescriptions:
    1. Visit the Sav-Rx website and enter the Group number on your ID card and your zip code. Present your Sav-Rx ID card at your pharmacy.
    2. Sav-Rx mail order pharmacy
      There are three options for sending in a prescription for mail order:

      • Ask your doctor to send the prescription electronically to

      • Ask your doctor to fax the prescription to (402) 753-2890
